Rather disappointing: The required data was provided on screen but never sent to my e-mail address (I checked to ensure it hadn't gone into my spam box). The only way to retain the info was to print from the web site screen, which resulted in the loss of all the graphical enhancements (i.e. no colour coding, tick boxes etc.).
