My call with Chapter Medicare Advisors resulted in a seamless signing up for Medicare Plan B. Chapter provided a concise and comprehensive overview of Medicare Plan A, Plan B, Supplemental Coverage and Plan D along with providing form SSA 40 and SSA L564 both needed for applying for Medicare Plan B coverage. Following Chapter's review of my doctors and medications, Chapter during our ZOOM meeting determined and submitted an application for Supplemental Coverage best suited for me, sent an application for Plan D while advising a myriad of information relating directly to monthly premiums and annual deductibles for these coverages.
