I received the proper, most current forms FD-258 cards. Several other sellers have these but they are usually the expired versions. If you are going to submit prints, make sure you submit the most current forms to avoid delays or rejections.
I was amazed how fast I received them from the southwest US to the Midwest in about 48 hours after placing my order! I ordered on Thursday afternoon and received the cards on Saturday afternoon! WOW!
Hats off to NGT for having the proper, current forms at a great price and very fast shipping.
