I don't really have an issue. It's good.
Now, to be very good, the yearly questionnaire process could be more frictionless. For instance, for the customer to log in to their website, see a clear notification about the need to fill out the yearly questionnaire, and have that link take you to a predefined form to fill out.
It would be more straightforward and frictionless than having to receive and answer the questionnaire as a comment. It would also make it easier to save each form for each customer instead of having the answers across one or several comments.
