there was a question that was phrased as "under the care act you should always..." and then the correct answer was assume the person can make decisions for themselves... that is either incorrect, poorly worded or doesn't account for iterations

Hello Horatio, Thank you for taking the time to leave us a review and your feedback. I have been in touch with our content team to ask them to have a look into this. They have looked through the course and they think the question you may be referring to is 'Under the Mental Capacity Act, you should always.." They have come back to me and confirmed that under the Mental Capacity Act we must always assume that an adult has capacity to make their own decisions. However there are some iterations where this isn't the case, such as if it has been deemed by a formal assessment that the adult lacks capacity. This must be formally assessed and it is illegal to just assume that someone doesn't have capacity. Although the answer is correct they are going to tweak it so that it's 'Assume that an adult has the capacity to make their own decisions unless it has been formally determined otherwise'.
