the 8th question suggested that when witnessing harassing behaviour, the correct response was to personally step in and call it out, and that to report the behaviour to HR or line manager was the wrong thing to do. If the behaviour is part of a pattern, this may not be picked up or acted on if the behaviour is consistently not reported, especially if the victim is a subordinate.
