“I felt there was too much on non LOTO subjects. I was not interested in risk assessments nor hierarchal systems. Also, from experience there was a step missing from your LOTO procedure which when missed is what causes accidents. That is to Check that the equipment has been correctly isolated after being locked out. If a supply was missed during the preperation (as I have seen) then eclecticutions have occured because they did not check the equioment was dead either by turning it on or checking with a multimeter. Given that of all the incidents I am aware of that have happemed during maintenace it was becuse they did not check the equipment was actually isolated. I am very disppointed that this was not part of the training.”
