“Please check question: How should a RIDDOR report for a dangerous occurrence be submitted. I selected "Online" and was given a big green tick with following text "Correct. Incorrect - a dangerous occurrence should be reported to RIDDOR as soon as possible, which means dong do via phone". My colleague selected "By Phone" and was given a big red cross with following text "Incorrect. Correct - Well Done!". I ended up getting 10/10 correct and he was given 9/10 correct. I have screenshots if required.”
