Thank you for your feedback and we are sorry for any inconvenience this caused. Unfortunately, with warranty claims, we do need to follow the policies set by the manufacturer. We spoke with someone from your company on June 29th and advised her that this form had not been returned. She said she would have the installing technician complete the form and we re-sent it to her that same day. To our knowledge it has not been sent back. If you have the warranty form completed, please send it to support@voomisupply.com and we would be happy to assist you in submitting the warranty claim.
