A massive screw with a three ball handle that exerts a spreading action on the two shaped nuts - acting like a bottle jack but upside down. The end cap is a press fitted ball joint, so it will swivel to adjust to uneven surfaces but cannot be used for pulling, in other words, the nut halfway up has to form the fixed part of the mechanism and the screw delivers pressure away from it, as in a Scandinavian face vice for example. This is a huge screw - the three ball handle is 9⅜" (nearly 24cm) across. Ideal for building into a sturdy frame to make a press - veneering, laminating, wood block printing, apple juice...?

Its a nice piece of kit that I'll keep as I like the aesthetics, but a couple of points. Mine arrived with a casting defect ( a hole ) in the handle which had some sharp edges. Filled with JB Weld to fix. The vertical handle doesn't rotate but the ball on the opposite end acts as a counterweight and allows it to spin nicely. The end cap with the ball joint does not have fixing points on it. If, like me, you wish to use it in a press, the plattern or whatever it is pushing cannot be retracted. Some holes will need to be drilled for screws or some sort of bracket made up. It states this in the description but I didn't think of retracting the plattern after pressing as "pulling" so think about your use case more than I did!
