On the Rainbow Medical Centre third interview I found the processes to be interesting the testing process for language and visual learning understanding didntt reveal anything I didn't already know about my thinking. At school and doctors I went through similar tests on my cognitive ability but nothing came of it. For the tests on subjective experience knowing what stimulus causes what reactions on a concious cognitive level. I found they didn't adequately analyse emotions throughout a human lifetime and more specific questions to get what they need to know. For example how sitting still for long periods of time has an affect if its done for hours. Do the questions analyse autism, ADD, dyslexia, dyspraxia and dyscalculia I felt I had to be the one too add them in. It could be nice to have the client bring in their own notes. I also didn't feel like I knew these people well enough to talk to them properly without pausing constantly.

The ADOS-2 (Autism Diagnostic Observation Schedule, Second Edition) is a semi-structured, standardised tool used to evaluate and diagnose Autism Spectrum Disorder (ASD). It is widely regarded as the “gold standard” for autism assessment. However, it is important to understand that the ADOS-2 is just one step in a seven-step diagnostic pathway, and its role is specific and focused. As a standardised assessment lasting approximately 1.5 hours, there are limitations to what it can explore. The ADOS-2 is specifically designed to assess the following areas: Language and Communication Reciprocal Social Interaction Imagination Restricted and Repetitive Patterns of Behaviour and Interests Bringing Notes to an ADOS-2 Assessment You asked about bringing notes to the session. Patients do not typically bring notes to an ADOS-2 assessment, as the assessment is designed to observe and evaluate natural behaviours and responses in a structured setting. The purpose is to observe how individuals interact with the examiner and respond to structured activities in the moment, without relying on prepared or rehearsed information. This ensures the assessment remains valid and reliable. Why Notes Are Not Used: 1. Natural Interaction: The ADOS-2 is meant to evaluate spontaneous behaviours, social communication, and flexibility of thought. Bringing notes could interfere with the authenticity of these observations. 2. Standardisation: The ADOS-2 is a highly standardised tool. Allowing notes might introduce variability that could affect the validity of the assessment. 3. Focus on Real-Time Observations: The clinician observes behaviours such as eye contact, tone of voice, turn-taking, and responses to social cues in the moment. Notes would not contribute to these dynamic interactions. Assessing the “Whole Self” You raised a valid concern about the ADOS-2 not exploring the “whole self.” It’s important to note that this is addressed as part of the entire diagnostic pathway. For example: - The Autism Diagnostic Interview (ADI-R), which lasts approximately 2.5 hours and is conducted with your chosen observer, delves deeply into your history, behaviours, and development, providing essential context. - The preliminary screening conducted at the start of the pathway is another step towards forming a complete picture. - These components, along with the ADOS-2 results, will be reviewed and discussed in detail at the multidisciplinary team meeting. This meeting allows all clinicians involved to combine their individual assessments and observations to form a comprehensive understanding of your strengths, challenges, and needs. Relationship with the Assessor You also expressed a desire to get to know the ADOS-2 assessor better. The reason this is not typically done is that the ADOS-2 aims to observe how you naturally respond in a new or unfamiliar social situation. Building a close rapport with the assessor beforehand could influence certain behaviours and responses, potentially affecting the objectivity of the assessment. By keeping interactions limited before the session, we ensure the assessment remains fair, valid, and reliable. That said, the assessor will always aim to make you feel comfortable and supported during the session, even without prior familiarity. Your well-being during the assessment is a top priority. Further Discussions and Next Steps Finally, many of the topics you raised—such as understanding Predominantly Inattentive Presentation (commonly referred to as “ADD”)—can be discussed in greater detail during your consultation with the psychiatrist at Step 4. This is an opportunity to address broader questions and ensure all areas of concern are explored fully.
