Even though the guidelines are really easy to follow, it does cover some cases like the c(11) category where the admitted people is paroled and they're able to apply for an EAD. Your guide indicates that an I-94 should be attached to the EAD renewal package. However, aliens in this category are not given the I-94 at the ports of entry.
